Welcome to our practice! 

During your first appointment, Dr. Daly will do a complete exam, which includes a screening for periodontal disease and oral cancer. He will discuss any concerns that you may have about your dental health.

Prior to your first appointment, we ask that you complete a Health History Form. For your convenience, you can print the forms below, complete at home, and give them to our receptionist when you arrive for your appointment.
